Completed registrations forms should be sent by fax to Lori Witting at 906-487-3101, or sent by mail to Lori Witting at the address provided above. For CIWIC-AIC and CIWIC-NM, a $250 deposit must be included with the registration form. For CIWIC-IP, the $250 deposit will be collected after proposals have been reviewed and acceptances finalized.
